NEM HEALTH CUSTOMER CARE

Introduction:  The HomeReach Plan is a healthcare insurance product offered by NEM Health Limited (“NEM Health”) to enable individuals in the diaspora to purchase health coverage for their loved ones residing in Nigeria. By purchasing or enrolling in this plan, the sponsor and beneficiary agree to be bound by the terms and conditions set forth herein.

Definitions

  • Sponsor: The individual residing outside Nigeria who purchases the HomeReach Plan for a beneficiary
  • Beneficiary: The individual residing in Nigeria who receives healthcare coverage under this plan.
  • Plan Tier: The level of coverage purchased -Tulip plan, Hazel, or Ivy plan
  • Coverage Period: A 12-month period starting from the date of activation.
  • Network Providers: Hospitals, clinics, and diagnostic centers accredited by NHIA for service delivery.

Eligibility

  • Sponsors must be Nigerians residing abroad, aged 18 years or older.
  • Beneficiaries must reside in Nigeria and be aged between 0 and 79 years at the time of enrollment.
  • Each beneficiary must be registered with valid identification and medical history (if required).

 Coverage Scope

 The plan covers only healthcare services specified in the plan brochure and subject to the selected plan tier.

  • Covered services may include General and specialist consultations, Diagnostic investigations, Prescribed medications, Hospital admissions, Surgical procedures, Emergency care, Telemedicine services, dental care, and optical care
  • Services outside the plan scope are not covered unless explicitly stated.

 Exclusions

The plan does not cover:

  • Pre-existing conditions (within the first 3 months unless stated otherwise)
  • Cosmetic procedures
  • Infertility treatments
  • Organ transplants
  • Experimental or non-approved treatments
  • Chronic disease care not disclosed at the time of registration
  • Other plan exclusions stated in the product brochure

Payment Terms

  • All premiums must be paid in full at the time of purchase through approved payment platforms
  • Coverage becomes active only after payment confirmation and verification of beneficiary details.
  • Premiums are non-refundable after plan activation.

Claims and Access to Care

  • Beneficiaries can access care at NEM Health-accredited providers using their digital ID or NEM Health card.
  • All services must be pre-authorized unless in emergency cases
  • Unauthorized treatment or treatment at non-network facilities may not be reimbursed.

 Plan Validity and Renewal

  • Each plan is valid for 12 months from the activation date.
  • Sponsors will be notified 30 days prior to expiration to renew the plan.
  • Renewal requires full payment of the applicable plan tier.

 Upgrade and Downgrade Policy

  • Plan upgrades are allowed at renewal or mid-term with approval.
  • Downgrades can only take effect during renewal cycles. Privacy and Data Protection
  • NEM Health complies with the Nigeria Data Protection Act (NDPA).
  • Sponsor and beneficiary data will be used solely for plan administration and will not be shared with third parties without consent, except where legally required.

 Amendments and Termination

  • NEM Health reserves the right to amend these terms with prior notice.
  • Plans may be terminated for fraud, misrepresentation, or abuse of coverage.
  • Sponsors may voluntarily terminate plans but are not entitled to refunds post-activation.

 Governing Law

 This plan is governed by the laws of the Federal Republic of Nigeria and subject to regulation by the National Health Insurance Authority (NHIA).
